We have constructed lists of sample shift patterns (rosters, rotas) for continuous work. They are available in the following formats:

Microsoft Excel spreadsheet
Microsoft Word document
RotaBase (a database of rotas that can be opened by our two software products, RotaBase and RotaView)

If your requirement is for one team to be rostered at all times, 24 hours a day, 7 days a week then there are two common methods:

4 teams
5 teams - usually within an annualised hours agreement

Sample rotas are based on 8 hour, 12 hour or a mix of 8 and 12 hour shifts. When there is a mix of 8 and 12 hour shifts, the 12 hour shifts are typically rostered at the weekend, to maximise weekends off, and 8 hour shifts during the week.  There are also examples based on full rotation of shifts within each working block ("continental" style) and alternating blocks of the same shift.
